I would hesitate to call it sad for the kid. Kids that age are real brats when it comes to getting attention. Someone on ONTD posted this improved version of that Vine.

You have to watch the whole video. The boy was acting up the whole time, even before the ceremony started. They let him run around, he kept interrupting people giving speeches. At one point, he's crying for his beanie (Mariah tells someone he's asking for his pacifier).
He was yanked away three times. The first time happens when the cover is being removed; both kids are right there. Monroe is behaving, but Morrocan crawls over and sits on the star. Mariah tells him you can't sit on it, annnnndddd here comes a mini tantrum. The nanny moves him over. After the cover is removed, the twins are standing on the star. Morrocan has the plaque. Mariah calls to the nanny ("Wendy, help") and she takes Morrocan away (he accidentally grabs the hair of a woman next to Mariah, his first grab attempt). Monroe gets to stay. You can here him screaming and losing it in the background.
That vine happens at this point. He comes back, rubs his face in her breasts, moves around, rubs his face against her leg. Settles down against Mariah, gets taken way. Now, this is when you notice something. He seems to have soiled himself. Also, he's wearing a pull up diaper.
The third incident happens a couple of seconds later. Both kids come back. Somebody accidentally nudges Monroe, who accidentally nudges Morrocan. He shoves her away. And then he sits down in front of Mariah, kind of fiddling with his pants. Mariah finally notices his wet pants. And tells her bodyguard and members of her management. And this is when she says "where's the nanny? Where is the nanny?" I think somebody is getting fired.
This is not the first time Morrocan has done this (tantrum in pubic). And I'm just really surprised by how nonchalant Mariah and her team are about his behavior. He literally shit his pants after the first time he got taken away and nobody noticed or did anything. I'm super curious what happened to the nanny at this point. Because she is nowhere to be seen.
Isn't four a bit too old to be still using a pacifier? Both of the twins do, there are pictures of them sucking on one during that month long vacation with Mariah's new boyfriend.

Four is really old for a pacifier, as well as wetting your pants. However, I get the feeling that the nanny can't offer any tips and tricks to Mariah when it comes to child rearing, so this is how it goes.

Since Mariah can marginally function as an independent adult, I can't imagine what she'd be like as a parent.

I think I have a better understanding why the Carey household goes through nannies like Kleenex. Where can I see the whole video? I remember being worried for CMJ's children because they were rich vagabonds with a crazy father. But it appears as though his children will be okay in spite of him. Hopefully there is an adult in the circus who is caring for the twins.

Here's video. He's comes back after he is dragged away and you can clearly see he has wet himself at about 5:55. Starting about 5:10 you see some obstructed views of what look like wetness and when he sits down you see Mariah react like she notices he's wet. And since we're going there, and I didn't want to be the first one to do it, it is just downright bizarre how she does not react at all when her children are grabbing at her. She doesn't comfort them, she doesn't discipline them, she barely acknowledges that they exist. The way she doesn't let Moroccan break her focus from being photographed is a little chilling.

So maybe the pacifiers are used to self-soothe?

Holy crap. Mariah is so disengaged from her kids; it's like she sees them as tiny little fans of hers, little "lambs". Not her own children, and not even children with whom she has any kind of attachment. And it's so obvious that all they crave is attention from her, and all she craves is attention from everyone else.

I give her a pass on maintaining her composure since this is a professional gig. And, this is certainly a time when the nanny should be taking over. However, I am disturbed by how out of control Moroccan is. I wonder if Mariah undermines any nanny's attempts to discipline him? I could totally see that being a problem in the household. And where is Nick Cannon in all of this?
