I have a couple:
The Hurt Locker: William James is having a bad day and takes a swig from a whiskey bottle, placing said bottle on the floor. Then he goes to turn out his light, and the bottle is on the table beside him.
Avatar: This might be more of a "I don't get it" thing, but (spoilering): <<Why wasn't Trudy thrown in the brig along with her fellow mutineers? She refused to fire on the Naa'vi and returned to base instead, therefore refusing to follow orders.>>

Shalamar, that's a good question. Maybe they weren't able to establish that particular action and so there couldn't be punishment?

Shalamar, I noticed that too. I fanwanked that since the mission was successful her gunner decided not to rat her out.

Avatar had big holes, if you ask me.
Spoiler:
The whole reason why the army couldn't storm the planet was due to the fact that the Na'vi and the terain of the planet were crazy. It even opened with the main character being assumed dead, after some crazy cat-dog beasts fire on his ass, sending him running for his life.
Suddenly, at the end of the movie, the earthlings come with bombs and army, and it appears it was easy peasy to whipe out Na'vi.
