I think one of the worst movie mistakes I've seen (in modern film history anyway) was the Incredible Moving Tattoo in that hideous film, Ransom. The title of this thread says it all: they just didn't care enough to check continuity.
ETA: I just saw Darwin Awards and in one scene, Winona Ryder is wearing boots with a heel and then they show her wearing a completely different type of boot (flats) right afterwards. I think she must have put them on cuz they were comfy and then no one checked again when they were ready to finish the scene. I hate stuff like that.

I remember years ago seeing Don Juan DeMarco and at one point there's supposed to be very earnest scene between Johnny Depp and Marlon Brando. As I was sitting there watching, this thing kept bobbing into the top of the frame until finally you could pretty much see the entire boom mike above them. There was a collective gasp in the theater then everyone started laughing.

According to Roger Ebert, who mentioned it in his Answer Man column, that's a projectionist mistake in not framing the image correctly, not a filmmaker error.

I just saw The Queen, and two things jumped out at me. First, the Blairs were shown in bed in two different scenes. And the beds were different in the two scenes. Now I could have missed something there. Maybe they were visiting somewhere in one of those shoots. But the second mistake is harder to explain. When the queen and Charles are going to meet the hunting party, she puts two dogs into the vehicle. When she decides to walk back, three dogs get out of the car.

i know I only watched The Queen a couple days ago but my memory is a wee bit fuzzy on the timeline, but perhaps the first shot of the Blairs was when they were in their old house, and the second was after they moved into the official Prime Minister's residence. Can't explain the dogs though...there seems to be varying amounts of dogs throughout the movie...how many corgis does she have anyways?

I don't know if this really qualifies as a mistake per se... but it's always REALLY bugged me. I just rewatched the movie St. Elmo's Fire the other night: There is scene at the beginning of the movie where Kirby Keger sees Dale Biberman at the hospital for the first time since graduation. He's telling her he's a lawyer (but he's in a waiters outfit)... Anyway, an emergency happens so she says sorry, I have to go... she swoops up a child and says something to the effect... 'your mommy will be alright' in nice hushed tones and the proceeds to bring him into the room where his mom is screaming at the top of her lungs in pain and blood is everywhere. Yea--that won't traumatize a child! Watching your mom writhe around in pain! See everything will be alright! Your mom will be just fine... now let's go watch her almost die! That scene has always bugged me.

In Harry Potter: Prisoner of Azkaban, during the Quiddich scene, Harry's scar (on his forehead for those of you out of the loop) kept changing places - from left to right sides and back again. Like every.single.cut. It was so annoying. Just put the damn scar in the middle so it won't be so noticable, fer cryin' out loud! That isn't the only thing that is annoying in that movie. When the trio is in the Hagrid's hut with him, and the rock gets thrown into the window, the vase next to Harry breaks. In the scene where Hermione and Harry go back in time and we find out that she is the one who throws the rock, the vase breaks differently. I can't remember which one is which but in one scene the vase completely breaks and in the other, it only half breaks. It makes me nuts every time I watch the movie...yes I do watch it a lot.

I think, in editing, sometimes they will flip an image and that might explain it but god, with all the money they have in the CGI budget, you'd think they'd fix the scar thing.

I have two rather lame ones:
1) In A Fish Called Wanda - the scene where Otto is driving Wanda to Archie's so she can seduce him. Wanda flips down the visor mirror and applies what is clearly light pink lipstick to her lips. Cut to the scene where she is standing in Archie's doorway, and her lips are bright red.
2) In Grease - the final scene where everyone's dancing around singing the "We Go Together" song, while Sandy has on those tights satin pants and super-high red heels, there are several times where Olivia Newton-John has removed her shoes so she can better jump about, then the very next dance step, her shoes are back on.

In Talladega Nights, the older son's hair length keeps changing through-out the movie, I just noticed it when watching it today. And yes, I did just admit to watching Talladega Nights multiple times. I heart Will Farrell. (Where is the Embarassing Crushes thread??)
