Julia Roberts' accent in Mary Reilly was horrific! Johnny Lee Miller's American accent in Hackers was a bit off, but he's cute so I didn't mind. Heh.

Taye Diggs in How Stella Got Her Groove Back. He didn't even try half the time, and when he did, it was hilarious.

I remember thinking that he was just a bad actor (but not really minding because of the hotness) with strange ways of speaking. It wasn't until years later that I found out that he was from the UK. I assume his Scottish accent in Trainspotting was better than his American one in Hackers, but I cannot judge the Scots accent at all (Mel Gibson in Braveheart sounded fine to me!).

Another vote for Julia Roberts in "Mary Reilly." Painful to listen to.
I also hated Michelle Pfieffer's southern accent in "Dangerous Minds." There's one part where she says, "Oh!" in surprise, and it comes out like, "Aaaoooww!" It just makes me cringe.

Edit: Oh, I'm forgetting the worst one: Heather Graham in From Hell. It was so bad I can't believe they let her film the whole thing. It was dreadful. It was so horrible. She was so all-around unbelievable.

James Cromwell in L.A. Confidential. It took me several viewings to work out that he was the Irish Police Chief and not just the weird Police Chief. Oh man, I was watching this yesterday and the badness blew me away. Oh James, I like you and all, but that sucked. Hard.

I think it was in The Ghost and the Darkness where Val Kilmer had a British accent... Well, part of the time. I usually have to have it pointed out to me when someone's accent is off because I generally don't notice that stuff. But this got even my attention. In one scene the accent was there, in the next it was gone, then it was back again. I was kind of surprised because I've always heard he's rather obsessive about getting in and staying in character.
